Horizons Care Center in Eckert has been recognized as one of the 2025 Best Nursing Homes by U.S. News & World Report, earning a high-performing rating in short-term rehabilitation. 

This honor places the facility in the top percentage of nursing homes both in Colorado and across the country. Out of more than 14,000 nursing homes evaluated nationwide, only 2,152 earned the high-performing rating in short-term rehabilitation. 

Ratings are based on metrics including nurse staffing, patient outcomes and safety practices. Part of the Volunteers of America National Services family, Horizons Care Center offers both inpatient and outpatient rehabilitation, along with long-term skilled nursing care.
